Summary Proposal:
1. Salary: 83K
2. PTO: 15 days in addition to 9 paid holidays and 3 Con-ed Days
3. Con-ed: Unlimited but eventually will cap around 2K
4. Schedule: You set your own hours up to 40. Evenings are most needed, especially Tues-Friday
5. Insurance: We o?er Medical, Dental and Vision via UHC. We have Aflac for STD and
accident
6. 401K. Infinity matches 1% , employee contributes 3%
7. Equity: I would like to create a profit sharing plan that pays on a quarterly basis. We have discussed 2 plans. 1. a quarterly profit sharing plan that is based on collections. To be eligible the PT must collect at least 20% over their cost to Infinity. Profit checks will be between $250 and $1000 depending on collections. 2. Bonuses are regularly available for tasks that help Infinity. There are several examples: 1. Taking a student 2. Seeing a patient on Saturdays 3. Creating new referrals 4. Selling products the clinic offers
8. Annual salary raises are not guaranteed but bonuses are given for annual performance.
Infinity Performance PT is growing quickly. The vision for Infinity as it currently stands is
Ashburn being our center location with satellites in Middleburg and Tysons Corner.
Total PTs would be 6 to 7 in Ashburn, 2 in Middleburg and 3 to 4 in Tysons Corner. In addition
we have wellness services such as, massage therapy, Pilates, personal training and
nutrition to o?er our patients in each location. We have a 11 year lease with the option
to renew at year 7.

Therapists are expected to expand their learning and train for OCS or manual therapy certifications.
